Good Faith Estimates

Birdwalk Therapy Collective is a private-pay practice. If you do not have health insurance or do not plan to use health insurance to pay for occupational therapy or mental health therapy, you generally have the right to receive a Good Faith Estimate explaining how much your expected care may cost.
What is a Good Faith Estimate?
A Good Faith Estimate is a written estimate of the expected charges for health care items or services that are reasonably expected to be provided.
The estimate is not a bill. Actual services, frequency, duration, or charges may change if your needs or treatment plan change.
Who may receive an estimate?
You may qualify as uninsured or self-pay if:
- You do not have health insurance covering the service; or
- You have health insurance but do not plan to submit a claim or use that insurance to pay for the service.
Birdwalk does not bill commercial insurance directly.
When will Birdwalk provide an estimate?
You may request a Good Faith Estimate before scheduling services.
When applicable under federal law, Birdwalk will generally provide the estimate:
- Within one business day when care is scheduled at least 3 but fewer than 10 business days in advance
- Within three business days when care is scheduled at least 10 business days in advance
- Within three business days after receiving a request for an estimate before services are scheduled
A Good Faith Estimate may not be required when care is scheduled fewer than three business days in advance.
What will the estimate include?
The estimate may include:
- The client’s name and date of birth
- The provider or practice information
- The expected occupational therapy or mental health therapy service
- Relevant service or billing codes, when applicable
- The expected charge for each service
- The expected frequency or number of services, when reasonably known
- The period covered by the estimate
- Required notices and disclaimers
The estimate will be based on the information reasonably available when it is prepared.
Changes in services or cost
A Good Faith Estimate is based on the expected services known at the time it is issued.
The estimate may change if:
- The treatment plan changes
- The frequency or duration of services changes
- Additional services are requested or recommended
- A different provider or service becomes involved
- Information provided during the intake or evaluation changes what is expected
When appropriate, Birdwalk may issue an updated estimate.
Your right to dispute certain charges
If a bill from a provider is at least $400 more than that provider’s Good Faith Estimate, you may be eligible to use the federal patient-provider dispute resolution process.
You generally must begin the dispute process within 120 days of receiving the initial bill.

Sensory-informed design projects
Sensory-informed design, consultation, equipment recommendations, purchasing, installation, and related project work may be governed by a separate proposal, quote, scope of work, or service agreement.
A sensory-informed design proposal is not necessarily a federal health care Good Faith Estimate. Birdwalk will provide applicable project pricing and terms separately.
Authorization or funding through the Self-Determination Program is not guaranteed. Clients and families remain responsible for confirming authorization, funding, fiscal-management requirements, and payment arrangements.
